Comprehensive outpatient services include: Opiate Dependence Treatment Program (MAT using Vivitrol) Heroin Addiction Treatment Program (MAT using Suboxone) Methadone Conversion Program Naloxone Program (free prescription for overdose medication) Counseling and Psychiatry Treatment Continuation Program

Provides individuals with disabilities the opportunity to be involved in community programs. Provides accommodations for youth, teens, and adults to participate in the following:

Administers Housing Choice Voucher program for income eligible households. Housing Choice Voucher program subsidizes part of the total rent. Housing Choice Voucher applicants are responsible for housing search and selection.

Provides free transportation to medical appointments for Lake County residents. Transportation is provided by volunteers who use their personal vehicles. Most vehicles have space to store walking aids, but do not include wheelchair lifts.

Serves as health center with variety of outpatient services. Provides Pediatric Services, Women's Health Services, Primary Care Services, and Dental Services. Pediatric Health Services: Provides complete care to children from birth to age 18. Services include: developmental assessments, hearing and dental screening, basic vision testing, immunizations, and lead testing. Women's Health Services: Provides a wide range of outpatient comprehensive services for women of all ages. Services include counseling, prenatal, and gynecological care. Primary Care Services: Offers a broad spectrum of personal and community services directed at individuals and families of all races, genders and ages who are unable to access the private medical sector. Dental Care Services: Provides primary dental care for children and adults, including: Dental Exams, Cleaning, Dentures, Digital X-rays, Fillings, Simple Extractions, Dental Sealants, Referrals to Orthodontics and Specialty Care, WIC Client Dental Screenings, School Dental Screenings, and Care for Pregnant Patients. (Payment for specialty care will be the responsibility of the patient.

Accepts furniture donations from individuals in the community. Distributes items to individuals in need for free. Notes: All pickups must be scheduled at least one week in advance. There is a suggested donation for pick-up service. All drop-offs require an initial call to ensure a quick unloading of items. This program does not provide free furniture directly to the public; must be referred by a caseworker to qualify.

Offers classes for adults to learn about individual and family health care. Topics include:

• Health conditions and prevention (diabetes, obesity, hypertension etc.)
• Accessing health care
• Health care advocating

Classes offered in both English and Spanish.

Offers low-cost internet service for low-income households. Includes free installation and in-home Wi-Fi.

Offers a tuition-based, Catholic college prep school for low-income students in grades 9-12. Students explore career options and create a path to achieve goals.

Provides immediate medical care to treat both adult and pediatric patients. Treats mild to acute medical concerns. Offers x-ray services, on-site lab, and online appointments.

Provides patients, survivors, and loved ones group support to cope with cancer diagnosis. Discussions range from topical, practical matters to emotional and spiritual struggles. Also offers spousal and family bereavement groups.

Offers the following transportation services for Waukegan Township seniors: Weekday Medical Transportation Provides curb to curb transportation for medical appointments within the township. Rides must be requested in advance. Shuttle runs weekdays only and is equipped with a wheelchair lift. Weekend Transportation Provides curb to curb transportation on weekends for travel within 10 miles. Rides must be requested in advance. Walking aids must be able to fold down for storage. Transit Card Issues a debit card to be used for personal transportation services. Funds can be used for any type of transportation service including taxis and ride shares.

Provides reservation-based, door-to-door transportation for certified ADA passengers. Serves individuals whose disability or health condition prevents them from using fixed-route service. Trips are provided within the same hours and geographic area as fixed-route service. NOTE: Passengers must be ADA Paratransit Certified. Call the Regional Transportation Authority (RTA) ADA Paratransit Certification program to request an application and schedule an in-person eligibility interview.

Offers reproductive and sexual health medical services, including:

• Abortion (medication abortion, procedural abortion, and follow-up exams)
• Birth control and emergency contraception
• STD, HIV, and pregnancy testing
• Hormone Replacement Therapy for LGBTQ

Provides prescription drug and sharps disposal boxes for outdated or unused prescription drugs. The collection boxes are located in the police department lobby. Items acceptable for the prescription disposal box include:

• Prescription medications, including controlled substances
• All over-the counter medications
• Medication samples
• Pet medications
• Vitamins and supplements
• Medicated ointments, lotions, creams and oils
• Liquid medication in leak-proof containers
• Homeopathic remedies

Items not acceptable for deposit in the collection box are:

• Needles/sharps
• Syringes with needles
• Thermometers
• IV bags
• Bloody or infectious waste
• Personal care products
• Empty containers
• Hydrogen Peroxide

Sharps Container available in the lobby for needle disposal. Sharps include:

• Syringes
• Hypodermic Needles
• Needles attached to tubing
• Lancets

To properly dispose of needles, do the following:

• Must use an approved disposal container that fits inside the disposal bin
• DO NOT deposit any loose sharps into the collection bin

Provides one-time financial assistance for Township residents facing an emergency situation. Provides help for emergencies such as eviction, past-due mortgage/rent, or utility disconnection.
