Provides assisted transportation and curb to curb service depending on client's needs. Clients can travel up to ten miles and allows for destinations outside of Lake County. • Needle Exchange - allows individuals to exchange used needles for sterile ones
• Peer Support - Peer Recovery Specialists always travel as part of the outreach team
• Treatment Navigation - connects individuals with resources for jobs, housing, legal assistance, social service registration, etc. Provides transportation to appointments when available.
• Snacks and personal care items
• Printed resources - informational materials with recovery resources

Matches volunteers with seniors for grocery shopping assistance and to deliver groceries. Coordinates grocery store selection, shopping list, and payment method. Note: volunteers do not transport seniors to the grocery store for shopping assistance.

Provides mental health counseling and psychiatric services. Offers individual, couples, youth, and family therapy.

Provides curb-to-curb transportation to older adults and people with disabilities of West Deerfield Township. Priority given to medical appointments, but non-medical transportation may be available. Participants must be able to get to and from van without assistance of driver. Caregiver may ride with participant. Offers up to two round trips per week. Taxi-Cab Coupons: Provides taxi cab coupons for seniors and adults with disabilities. Coupons may be used for one way rides to and from Highland Park Hospital and are valued at $4.25.

MEDICARE is a federal health insurance program consisting of two segments: Part A: Provides hospital insurance to help pay for inpatient hospital care and certain follow-up care upon departure.
