Emergency Shelter Provides emergency shelter for boys and youth identifying as boys, ages 14-17 years. Serves eight boys at a time with maximum stay of 120 days. Offers support and guidance to plan next steps. Transitional Housing Provides transitional living program for young adults of any gender, ages 18-23 years. Serves six young adults for up to two years. Also offers two emergency beds for young adults.

Provides education to the public about sexual violence. Aims to spread awareness and prevent future violence from occurring. Offers presentations for staff, students, and parents. Also offers trainings for professionals who work with survivors of sexual assault.

Hears all criminal and civil cases, including:

• Felonies and misdemeanors
• Traffic tickets
• Family law
• Probations
• Juvenile cases
• Appeals of small claims

Provides affordable health insurance for children regardless of immigration status or health condition. Covers doctor visits, hospital stays, prescription drugs, vision care, dental care, and eyeglasses. Also covers regular check-ups and immunizations (shots). Special services are also covered, including medical equipment, speech therapy, and physical therapy.

Provides supportive services for family caregivers of an enrolled veteran. Offers the following two programs based on level of care (see eligibility guidelines below): Program of General Caregiver Support Services (PGCSS)

• Individual support related to the care of the veteran
• Skills training on caregiving for a veteran's unique needs
• Respite care
• Peer support mentoring
• Training and support through in-person, online, and telehealth sessions
• Referrals to available resources

Comprehensive Assistance for Family Caregivers (PCAFC) - Provides all services listed above and the following:

• Mental health support
• Virtual Psychotherapy for Caregivers Program (VPPC)
• CHAMPVA (if uninsured)
• Monthly stipend
• Beneficiary travel

Offers various educational programs for Lake County youth, including:

• 4-H Youth Development: Introduces youth to potential careers and offers training.
• Commercial Agriculture: Works to address the needs of various local producers.
• Consumer Economics: Offer tips, strategies, and insights into managing finances.
• Horticulture: Encourages individuals to learn about the benefits of flower and vegetable growing.
• Nutrition and Wellness: Food safety to nutrition to chronic disease prevention and management.
• Family Life: Connect with information and resources to assist with helping family members

Places children who have experienced neglect and/or abuse in a safe home. Other services include:

• Counseling services for children, foster parents, and birth parents
• Children advocacy in court
• 24 hour crisis interventions
• Foster home monitoring
• Referrals for birth parents to get help for issues that prevent them from being with their children

Offers transitional housing for Lake County families who are experiencing homelessness. Also provides case management services, including:

• Life skills classes
• Employment assistance
• Educational advocacy
• Financial stability

Provides outpatient mental health services for children, adolescents, and adults. Offers individual and group therapy. Also offers Intensive Outpatient Program (IOP) and Partial Hospitalization Programs (PHP) for adults with severe mental health issues. Provides individual and group therapy, and medication management. Treatment is three or six hours per day for four to five weeks.

Offers court-ordered classes for traffic violators, DUI offenders, divorcing parents, and community service hours. Offers the following courses:

• The National Safety Council’s Defensive Driving Courses: promotes awareness and safe driving practices

• Live Victim Impact Panel: for individuals charged with DUI, reckless driving, or other serious traffic offenses

• Substance Awareness Module: provides an educational approach to drug and alcohol abuse

• Behavioral Choice Module: provide juveniles and adults strategies to look at their behavior and how to make better choices in the future

• Family Parenting: discusses the effects of divorce on minor children and how to minimize the effects

• Probation Support Program: provides volunteer opportunity for court-ordered community service

Provides monthly financial assistance to help Township residents in need. Helps pay towards rent/mortgage, utilities, or medical expenses. Provides temporary aide while individual finds a job or waits for benefits approval.

Provides law enforcement, crime reporting, and emergency services. Offers administrative assistance with police reports and records.

Provides property assessment information on properties within Lake County. Other services include:

• Publishing both annual and supplemental assessment rolls
• Facilitates assessment appeals process in connection with Lake County Board Review
• Administration of all preferential exemption applicants

Offers monthly food distribution for individuals in need. Offers a drive-thru experience where volunteers load the items into the client's trunk.

Provides trauma counseling services for sexual assault, domestic violence, and child abuse.

Offers Medication-Assisted Treatment (MAT) for individuals with opiate and heroin addiction. Comprehensive outpatient services include: Opiate Dependence Treatment Program (MAT using Vivitrol) Heroin Addiction Treatment Program (MAT using Suboxone) Methadone Conversion Program Naloxone Program (free prescription for overdose medication) Counseling and Psychiatry Treatment Continuation Program

Provides permanent housing for victims of domestic violence or human trafficking. Provides counseling and other support services for all residents. Accommodates adults with or without children. Permanent Supportive Housing Provides affordable housing within a 37-unit apartment building for survivors and their families.

Helps with the adoption process for those needing an emotional support animal (ESA). Offers financial assistance for adoption fees, training, and care costs. Also helps individuals get an ESA letter from a licensed therapist.

Provides grants up to $60,000 to assist low-to moderate income households to complete critical home repairs, including:

• Roofing
• Windows & doors
• Heating & cooling
• Electrical
• Plumbing
• Lead Based Paint, mold and asbestos remediation
• Kitchens
• Bathrooms
• Flooring
• Foundation and structural repairs
• Wheelchair ramps
• Widening doorways
• Accessible kitchens and bathrooms

Also provides loans of up to $25,000 for repairs, accessibility improvements and other home improvements.

Offers a 24-month residential program for survivors of sex trafficking and their children. Follows a six-stage treatment plan to help survivors heal and become self-sufficient. Provides safety, basic needs, and individualized treatment, including: Therapy Addiction recovery Health and wellness Education Financial literacy Spirituality Parenting Job preparation

Satellite medical campus offering emergency care and outpatient medical services. The Ambulatory Care Center offers the following outpatient services:

• Physical, occupational, and speech therapy
• Imaging, MRI, and Mammography
• Laboratory services
• Sleep Center

Serves as a health center with a variety of outpatient services. Provides Pediatric Services, Women's Health Services, Primary Care Services, STI/HIV Program Services and Gender Health Services. Pediatric Health Services: Provides complete care to children from birth to age 18. Services include: developmental assessments, hearing and dental screening as well as basic vision testing, and immunizations, and lead testing. Women's Health Services: Provides a wide range of outpatient comprehensive services for women of all ages. Services include counseling, prenatal, and gynecological care. Primary Care Services: Offers a broad spectrum of personal and community services directed at individuals and families of all races, genders and ages who are unable to access the private medical sector. STI/HIV Services: Provide sexually transmitted infections (STI) and HIV prevention services. Services include: testing, treatment, and free condoms and other safe sex supplies. Dental Care Services: Provides primary dental care for children and adults. Services include: dental exams, cleaning, and referrals to Orthodontics.

Promotes a variety of youth social and physical programs. Also offers senior exercise classes and sports activities for people with a physical or mental disability.
