Provides home visits with registered nurses and health navigators to eligible clients. Offers assessments, screenings, health education, parenting education, and help with resources.

Provides drug users a fast-track to addiction treatment services. Allows individuals to call or walk-in to Lake County Sheriff's Offices to request help. Individuals may surrender any drugs or paraphernalia without risk of criminal charges. A deputy will immediately coordinate phone screening with the Lake County Health Department. Based on the evaluation, individual will begin outpatient or inpatient treatment. Provides a police escort for any individuals who cannot transport themselves.

Services include: Individual Counseling Family Counseling Group Counseling Tele-Counseling Access to 12-step support on-site Assistance signing up for Voluntary Self-Exclusion Program Free 1-year subscription to program blocking online gambling Access to financial/credit counseling Free smoking-cessation materials Also offers educational workshops with topics such as: Responsible gambling Problem gambling Athletes and gambling Seniors and gambling Teens/Young adults and gambling

Provides assessment and treatment for individuals with substance use disorders. Services include:

Also offers workshops and prevention services for at-risk populations.

Administers Housing Choice Voucher program for income eligible households. Housing Choice Voucher program subsidizes part of the total rent. Housing Choice Voucher applicants are responsible for housing search and selection.
