Pediatric Health Services: Provides complete care to children from birth to age 18. Services include: developmental assessments, hearing and dental screening as well as basic vision testing, and immunizations, and lead testing. Women's Health Services: Provides a wide range of outpatient comprehensive services for women of all ages. Services include counseling, prenatal, and gynecological care. Primary Care Services: Offers a broad spectrum of personal and community services directed at individuals and families of all races, genders and ages who are unable to access the private medical sector. Dental Care Services: Provides primary dental care for children and adults. Services include: dental exams, cleaning, and referrals to Orthodontics.

Offers individual and group psychotherapy for adults, children, families, and adolescents. Counselors work with clients to structure a treatment plan that addresses their needs. Hosts summer day camps with meals included. Provides enrichment activities, arts, and learning opportunities. Offers recreational activities, sports, club groups, and field trips.

Provides supervised residential setting for adults living with severe mental illness. Length of stay can be between 6 - 12 months. Offers residents help to understand their condition, develop coping skills, and improve their interpersonal and daily living skills. Services include: Medication Monitoring Psychiatric Care Linkage to Community Resources & life skills training ( funding, housing, vocational, health care, insurance & entitlements, budget management, cooking, housekeeping, socialization. laundry, personal care & recreation).

Provides rebates on a wide range of energy-saving improvements and conservation practices to residents. Available rebates include:

• $500 for an integrated boiler and hot water heater two-in-one unit
• $350 for a standalone hot water boiler
• 200 for a natural gas furnace
• $150 for a steam boiler
• $150 for a tankless water heater and an indirect water heater
• $25 for a smart thermostat
• $20 for a programmable thermostat

Weatherization Rebates:

• Air sealing 40 cents per cubic feet per minute (CFM) reduction Capped at $400
• Wall insulation 50 cents per square foot Capped at $400 per project.
• Attic insulation 30 cents per square foot Capped at $500 per project
• Duct sealing $2/CFM reduction Capped at $400

Offers counseling services for adults and children ages three years and older. Services include individual, family, group, and grief therapy.

Offers free classes for adults to improve in reading, writing, math, and conversational English. Offers the following classes: English as a Second Language GED testing and preparation Citizenship Adult Basic Education (reading, writing, and math) Computer literacy Job readiness classes

Coordinates educational services for homeless students. Acts as a link between family, shelter, school system, and community resources.

Offers case management services to families and connects them to necessary resources. Areas of need include: Parenting classes Family counseling Legal aid and immigration issues Healthcare referrals Domestic violence counseling referrals Housing referrals Referrals to other local social service agencies for financial and basic needs Referrals to substance use, problem gambling, and mental health treatment

Accepts furniture donations from individuals in the community. Distributes items to individuals in need for free. Notes: All pickups must be scheduled at least one week in advance. There is a suggested donation for pick-up service. All drop-offs require an initial call to ensure a quick unloading of items. This program does not provide free furniture directly to the public; must be referred by a caseworker to qualify.

Responsible for repairing and maintaining roads in the township, including:

• Roadside mowing
• Tree trimming
• Snowplowing and ice control
• Drainage, storm sewers, and culverts maintained to ensure proper flow of water
• Development and implementation of storm water program
• Street sign maintenance
• Dead animal removal

Provides services to individuals with severe disabilities so they can remain independent in their homes. Offers the following program options for independence:

• Personal Assistant (PA): Provides assistance with household tasks, personal care and, with permission of a doctor, certain health care procedures. PAs are selected, employed, and supervised by individual customers.

• Homemaker Services: Personal care provided by trained and professionally supervised personnel for customers who are unable to direct the services of a PA. Instruction and assistance in household management and self-care are also available.

• Maintenance Home Health: Services provided through a treatment plan prescribed by a physician or other health care professional. Other services include nursing care and physical, occupational, and speech therapy.

• Electronic Home Response: Emergency response system offered by hospitals and community service organizations. This rented signaling device provides 24-hour emergency coverage, permitting the individual to alert trained professionals at hospitals, fire departments, or police departments.

• Home Delivered Meals: Provided to individuals who can feed themselves but are unable to prepare food.

• Adult Day Care: The direct care and supervision of customers in a community-based setting to promote their social, physical, and emotional well-being.

• Assistive Equipment: Devices or equipment either purchased or rented to increase an individual's independence and capability to perform household and personal care tasks at home.

• Environmental Modification: Modifications in the home that help compensate for loss of ability, strength, mobility or sensation; increase safety in the home, and decrease dependence on direct assistance from others.

• Respite Services: Temporary care for adults and children with disabilities aimed at relieving stress to families. Respite services may be provided for vacation, rest, errands, family crisis or emergency. Services may include personal assistant, homemaker or home health.

Also provides specialized services for people with HIV/AIDS and/or traumatic brain injuries (TBI). The Community Reintegration Program helps individuals with disabilities who live in nursing homes move into community with the supports they need to live as independently as possible.

Coordinates educational services for students who are experiencing homelessness. Acts as a link between family, shelter, and school to serve the needs of the student.

Offers health literacy and educational workshops. Topics include cancer, cardiovascular disease, nutrition, osteoporosis, and sexually transmitted infections. Also offers mammograms and screenings for blood sugar, cholesterol, osteoporosis, and blood pressure. Screenings are offered every third week of the month. Other services include:

• Healthcare navigation and interpretation
• Transportation to health screenings
• Exercise classes
• Baby and infant diapers
• Public notary assistance
• Mental Health First AID

Provides monthly financial assistance to help Township residents in need. Helps pay towards rent, utilities, or medical expenses. Provides temporary aide while individual finds a job or waits for benefits approval.

Offers after school program for North Chicago Community Highschool students. Provides age-appropriate after school and summer activities for Lake County youth ages 5-18 years. Programming includes a snack, hot dinner, and homework help. Also offers the following:

• Homework assistance
• Health/athletic activities
• Outdoor activities
• Cultural arts
• Leadership development activities
• Science, Technology, Engineering, and Math (STEM) activities
• Career exploration
• Character development

Offers grief support groups for children, teens, and adults who are coping with the death of a family member. Includes groups for child loss, spouse loss, suicide loss, and age-specific groups for youth and young adults. Offers in-person meetings at various locations, and online meetings via Zoom.

Offers financial aid to help Township residents prevent loss of home, job, or utilities. Funds are available once every two years and provide help to meet basic needs, including:

• Preventing eviction or past due rent/mortgage
• Preventing utility disconnection
• Work-related expense necessary to getting or keeping a job (for example, uniform or licensing fees)
• Cost of prescription medication

Amount of assistance determined on a case-by-case basis. Program does not disburse cash as all funds will be paid directly to vendors.

Offers a community center for seniors to sign up for classes, trips, events, and support groups. Provides information and referrals for social services including transportation, counseling, and legal aid. Offers application assistance for the following:

• Low-income Home Energy Assistance Program (LIHEAP)
• SNAP
• Medicare and SSI
• Prescription drug discount programs
• Illinois Benefit Access Program
• Secretary of State programs (driver's license, Real ID, and car registration)

Offers affordable integrative healthcare for individuals of all ages. Outpatient health services include:

• Adult and senior health
• Infant, children and teen health
• Reproductive health, family planning, and prenatal care
• Dental care
• Behavioral health and counseling
• Substance use treatment services
• Medication-Assisted Treatment

Provides nutrition education, breastfeeding support, and supplemental food to eligible women and children. Offers assistance to buy healthy foods and baby formula. Also provides referrals to health care and community services.
