- Must be Lake County youth 18 years of age and younger who are experiencing psychiatric instability - Must demonstrate a medical necessity for this service based on clinical assessments and treatment plans

Provides a team of professionals to offer the following services: - Intensive therapy - Case management - Psycho-education support - Mental illness self-management - Development of interpersonal skills - Development of practical living skills - Identification and use of natural supports - Identification and use of community resources
