Call Community Action Partnership to schedule an appointment. Annual recertification required. Documents required: - Photo ID - Proof of income for the last 30 days - Social Security cards for all household members - Current utility bill - Copy of rental agreement (if renting) showing that utilities are included, the monthly rental amount, and landlord contact information - Proof of benefits if receiving assistance from the Illinois Department of Human Services
